Ananya Aesthetics LLC, DBA Ananya Aesthetics is the MSO for ZKN Medical Corporation.  Medical director Luani Lee, M.D.  

Self Care Members will pay $150/month.  This money goes into a "beauty bank account" to use on any treatment or product at Ananya Aesthetics.  Self Care Members agree to a monthly contract term and agree to be billed on a monthly basis. Termination of membership contract can be requested at any time, but all early cancellations will result in a $75 cancellation fee and all discounts given must be paid back to the company. Cancellations require a 30 day written notice by either party.  Free member services (mini treatment or Microneedling) do not roll over to the next month or year if the client does not use it, nor can they be transferred to another person.  There must be a minimum of 6 months in between free microneedling services and in the following calendar year.  If payment is late, late fees will apply.  Discounts are applicable solely to the initial microneedling or laser package or single treatment purchase.  Should you choose to inlude an additional treatment area, such as the neck, as an add-on to original purchase, please note that the discount will not extend to this/these add-on(s). The full price will apply to any areas beyone the initial treatment package. Self Care Members are subject to ZKN Medical Corporation/Ananya Aesthetics LLC's cancellation policy for any missed or cancelled appointments with less than 24 hours notice. Members are subject to ZKN Medical Corporation/Ananya Aesthetics LLC's Late Show policy.  Self Care Members are subject to ZKN Medical Corporation/Ananya Aesthetics LLC's No Refund Policy. Membership benefits cannot be combined with any other offer or promotion at Ananya Aesthetics. Inception of Membership will occur on the first (1st) day of the following month once initial payment is submitted, and Self Care Membership Agreement is signed & submitted to Ananya Aesthetics.  Other rules and restrictions apply.  Please see contract for those.
